Per request. Was a bit hesistant to upload this since there are three missing cap values. But socketing some values should get you close enough. magus femar did some socketing and ended up with some values that works well. See the notes below the layout. Note that I omitted the ferrite beads since they are overkill and I also omitted the direct output jack. Schematic is avaiable HERE.
